In 1974 each player wore their own high school helmet. There was also a circular All-Star logo decal on each side of the helmet. Sometimes the all-star logo was placed right over the regular high school logo. They played the North-South format from 1974-79 and later the Metro-Outstate format from 1980-82. The helmets shown are in the Metro-Outstate format.

1991-2010

Starting in 1991 each player wore their own high school helmet with their high school decal. Each player adds a "collage" of high school decals that they acquire from All-Star teammates. The trading of helmet decals and the"collage" of decals on the helmet is a tradition in most high school all-star games around the country.

2011-Present

North
South

In 2011 the game returned to a North versus South format for the first time since 1979. The North and South teams were divided by MSHSL competitive sections in all classes (1-4 South, 5-8 North; exception – 2AAAAA is on the North and 6AAAAA is on the South). All-Star players continue the tradition of the "collage" of high school decals that they acquire from other All-Star teammates.
